Default If there is a 'real' dog behaviorist or someone with some knowledge of german shephards could you please help me with my problem

I have a 5 year old male German Shepard. I have a list of things he does and can't seem to figure what to do.
first of all he lives with to adults two children 13, 20..we have a large yard that he can run in..I've had many German
shepherds all my life (i'm 52)..this dog can not stay still, he will chase airplanes from one end the yard to the other, If
I take him out while I'm working in the yard he is always under my feet, never lays down when he is in the house again
he will just walk all around If I tell him to lay down he will for two seconds and before you even realize it he is under your
feet again..Its very important you understand he is well treated and played with quite a lot with all of us.
that's problem 1

this dog never seemed aggressive with us and we always were convertible leaving children around him. then when he was about a 2.5 years
old, one day he was with my daughter and her friend in the back room and they were playing. this girl had known this dog quite awhile.
all of a sudden I here a short bark and my daughter tells me the dog lunged and tried to bite her friend..with no warning at all.. it doesn't seem to happen
a lot but has happened 3 or 4 other times with different people adults and children..my sister who plays with him often was in my daughters
room and petting the dog, all of sudden the dog again let out short bark and lunged at her..she has been around this dog since we got him at
8 weeks old. .I can take this dog out for walks around the block without a leach and he will stay with me even if there are other dogs or people.
when he was young I had know problem letting people come up to him and pet him..never a problem now I'm leery to have him around any people. It
seems like you never no what will set him off 99.9 % of the time the dog is great around people.

Now when we take him to the vet for checkups he goes crazy pulling the leach I have no control even using a choke chain he will pull full strength dragging me
or the wife In to the vets where he just circles and barks..also he did lunge at the vet when he was given a shot.. also he is scared of thunder to a point
where he will knock over a chair trying to get behind it .then again going someplace else knocking every thing over. One time while sitting in my recliner with my
feet up watching TV at one point I let my foot down towards the floor and it touched his back and he barked and almost bite me.

any help of suggestions please..
