I don't know if these are relevant, but his nipples are kind of dry/crusty:

My 7 year old yellow lab used to look just like your pictures. Scaly
enlarged nipples, hot spots, crusty itchy skin, and he was plagued
with yeast infections. We were constantly at the vet for ear wash,
antibiotics, and steroids for the itch. Nothing seemed to work and the
vet couldn't give me a definitive answer as to what was causing the
problem. The first thing was to change his food - and we finally
settled on a whitefish/sweet potato blend that seemed to work. One day
I was in the holistic pet store buying the food and got talking to the
woman who owns the place. She said 'i'll be willing to bet that he has
seasonal allergies and this has nothing to do with what you're feeding
him.' At this point I was willing to try anything. She hooked me up
with some holistic allergy relief pills. I hesitate to give you the
website for fear of being labelled a troll - I assure you I'm not - i
have no vested interest in this place and I'm sure there are other
companies around who sell the same stuff that you could check out.
www.naturespetherbs.com I also put him on low-dose Benadryl once a
day (with the vet's approval.) I also don't let him swim in water I
wouldn't swim in myself. His skin and coat have never been in better
shape and he hasn't had an ear infection or skin outbreak in the 8
months I've been using this treatment.

Two words: SKIN BIOPSY

Your vet can easily collect biopsy samples in the exam room using only local
anesthesia and have them sent to a pathologist for interpretation. You need
this information in order to go further with treatment. In fact the odds
are very good that this will give you a definitive diagnosis.

Sounds to me like your dog might have psarcoptic mange.
I and my puppy were infected with this awful mange back in 1970. At the time, this was very unusual for a human to be infected by their pet, so we were written up in medical and vet journals and papers. We also were the guinea pigs for lots of so-called expert cures. Unfortunately, to no avail, Pancho was never cured and was eventually put to sleep.
20 years later I was relating my story to an old farmer in Texas and he told me 'the vets don't know nothing. All you have to do to cure mange is to put some used engine oil on the sores till they go away. I guarantee it'll cure it."

I was in disbelief, but a neighbor friend of mine who had introduced me to the old guy informed me that sure enough his cocker had had it 15 years before and the old guy told him the same thing. The neighbor followed the old guy's advice and I was petting the 20 year old cocker as we spoke.

Just thought I'd pass this on.

Keoni.

my 2 year old German Shepherd has. I've taken him to 3 vets and a
dermatologist. 2 months and $1k later, all I have is an I don't know...

His problem started last winter, a red spot in his stomach, right where his
ribs end. A trip to the vet and a low dose of Hydroxazine (pills, green and
white), and it went away.

About 4 months ago, it came back -- with a vengance. The best way to describe
it is a 2" circle, the skin is very bumpy/wrinkly, hairless, and red. This
time it's accompanied by deep red/purple over the entire inside of his hind
quarters. (I can take some pictures and post if someone feels they'd be
helpful). He constantly scratches and licks at it. He also developed a few
puppy zits. About dime-sized, and when they pop they squirt white stuff. One
was on his side, one behind his ear.

In the 4 months, none of the symptoms have worsened at all, but also have not
improved at all. We've been to a bunch of vets, none of whom have any ideas.
Here's what's been tried so far:

Thought: Maybe it's the same thing?
Hydroxazine -- nothing.

Thought: Maybe mange of some sort?
A series of Ivermectin -- nothing.

Thought: Ok, maybe allergies?
Kept him out of his crate for all but ~2 hours a day in case it's plastic --
a bit better, but not really.
Benadryl -- nothing
Steroid (can't recall which one) -- nothing
Food change -- nothing

Thought: Ok, just reealy dry skin?
Moisturizer -- almost instantly turns the red spot and his inner thighs to
white, but it doesn't go away, and he still itches.
Aloe/cortizone/itch creme -- turns it white, but again, nothing

Sorry for the long book post, but I'm kind of desperate. If anyone has any
thoughts, please, help!
