Probably. But without knowing more, examining him, etc., it's really
impossible to know.

Yes, it could be CHD, etc. But it's more likely to be soreness.

Is there anything we can do to help the pain?

Get your dog in shape. GRADUALLY!
That means a little hiking and swimming, on a daily basis if possible,
until he's ready for a whole day of it.
If after a week or so of daily exercise (hiking and swimming, if
possible), he's still expressing some pain or unsteadiness, it's
probably time to see your vet.
