Default dog toenail split in half, not healing up

hi,

It was the middle toe on one of his hind feet. It split right down
the middle, and it
smelled rotten. the dog then chewed it off, and he got the base. The
toe swelled up,
and the vet ordered a round of antibiotics for him. Didn't help. he
kept getting
the collar off. the swelling went down a little, but the rotten smell
coming from the toe
itself came back. Back to the vet. another round of antibiotics.
Thsi time the vet
said to put a bootie on the foot, but that makes everything even more
moist.
he's just a few days into the second round of antibiotics. So now
we're back to
the collar with me sitting around watching him all the time.

There are still bits of toenail that are working their way out of the
toe, and a red fleshy
part that looks like it wants to heal up, but isn't. I cut the hair
around real short.

The two adjacent toenails have slit underneath, but the nails are
still intact. I really hope
they don't split too. The vet just said to give him vitamins.

Has anybody heard of this, could this be some kind of toenail
disease?

He's a mixed breed: G. shephard, chow, and pit bull. 95 pounds.
